Project: Twelve (12) inch deep concrete repairs and paver system reconstruction

Scope or work itemized below
Reconstruct entire pavers system 5 x 140 replacing approximately 400 broken pavers
Cut and replace broken concrete and curbing

Concrete repair process:
 Sawcut and remove all concrete to approved refuge site
 Install 2" cushion sand pad
 Form area –curbing
 Install 3/8" rebar on 18" centers both ways
 Pour 12" concrete 6,000 psi concrete so the City Fire Trucks can have
    access within 24 hours
 Broom finish

Paver System reconstruction process:
 Set up traffic control devices
 Remove existing pavers
 Rebuild Sub-grade with dry concrete patch mix
 Install 1" cushion sand pad (use existing pavers if still there when
    project starts -purchase additional pavers as needed)
 Reset paver system
 Compact with plate compactor
